Steps to Purchase Childcare Services at the Annual Meeting

In order to receive care, Parent(s)/Guardian(s) must:

1. Sign the release included on the purchase form.

2. Provide any necessary medical and allergy information. Jovie staff cannot dispense medication, including sunscreen and bug spray. Parents must attend to all medication needs and applications prior to or during the event.

3. Remain at the event site while child is under Jovie care and provide a cell phone number where you can be reached in case of an emergency.

Sign In: All parents must present a photo ID or driver’s license at sign-in. A tag will be placed on each child’s back with his/her name, Parent/Guardian name, and a wristband will be provided if your child has an allergy.

Pickup: Children must be picked up within 15 minutes of the end of the event. Only the Parent (s)/Guardian(s) signing the child in may pick up the child. The Parent/Guardian ID must be presented and will be matched to the child tag. If a different parent/guardian must pick up the child, complete the “Alternate Parent/Guardian Authorized for Pickup” section at the bottom of the page. This person must also provide the Parent/Guardian tag at pickup. If a parent/legal guardian is late in picking up their child/s, they will incur a fee to provide for the additional service provided by Jovie caregivers. 

Photography: Jovie may take photographs of the children during the event solely for the purpose of providing updates to families. In addition, Jovie Philadelphia & Wayne may take photographs of children and adults during the event for use in its newsletter or similar [internal] materials. By signing this Release Form, I acknowledge and agree to the taking of photographs for these purposes.

By signing the purchase form, the parent/legal guardian of the minor child(ren) listed, on behalf of parent/legal guardian, their family and their child(ren) hereby release and agree to indemnify, defend and hold harmless MKTLI INC d/b/a Jovie of Philadelphia & Wayne, its affiliates, franchisor, subcontracted franchisees, and parent companies, and their officers, directors, sitters, employees, and agents (the “Releasees”) from any and all liability, losses, claims, damages, thefts of or damage to personal property, personal injuries (including death), costs, or expenses of any nature (“Claims” ) arising from or related to Releasees’ negligence or the event services. The release shall not apply to the extent that such damage or injury arises out of a sitter’s or Releasees’ gross negligence or willful misconduct.
