Ischemic stroke remains a major cause of death and disability, with mechanical thrombectomy often failing to prevent long-term deficits due to secondary ischemia-reperfusion injury. A key contributor to early-phase damage is blood-brain barrier (BBB) disruption, particularly through increased transcytosis in endothelial cells. Major facilitator superfamily domain containing 2a (MFSD2A), a critical regulator of caveolar transcytosis, is downregulated following stroke, leading to excessive vesicular trafficking and edema.
